Introduction: Reverse sural artery flap has been used effectively to cover defects of distal leg, ankle and foot. This flap is easier and quick to harvest and is a versatile flap in terms of extensive mobility and reliable vascularity and, in many instances it has obviated the need for free tissue transfer. Methods: This is a retrospective study of sural flaps done at the Department of Plastic Surgery and Burns in Tribhuvan University Teaching Hospital over a period of 4 years from April 2011 to March 2015. It included 21 flaps carried out in 21 patients with soft tissue defects at distal lower leg, ankle and foot. Results: Among 21 patients, 10 were men and 11 were women with an average age of 41 years (17 – 81). Successful flap coverage was obtained in 16 cases out of 21 (76%) which included 10 cases of complete survival (48%) and 6 cases (28%) of marginal flap necrosis. Four cases (19%) had partial flap loss and one case (5%) lost flap completely. Conclusion: we found that reverse sural artery flap has good reliability with low incidence of complications and is a versatile flap for the reconstruction of defects in distal leg, ankle and foo

Introduction: Split thickness skin graft (STSG) is a commonly carried out procedure in granulating wounds. The graft may be applied directly on the granulation tissue (without scrapping) or it may be applied after complete removal of the granulation tissue (after scrapping). Surgeons are divided on this issue. Methods: A single blinded, randomized, controlled trial was carried out between November 2017 and December 2018 in the Department of Plastic Surgery and Burns, Tribhuvan University Teaching Hospital, Kathmandu, Nepal. The study consisted of 62 patients who were divided into two groups. Group A consisted of patients undergoing removal of granulation tissue and Group B included patients on whom the skin graft was directly applied on the granulation tissue. On the 10th post-operative day, assessment of the graft take was done. Results: There was no statistically significant difference in graft take between group A and group B (p=0.59). The average drop in hemoglobin levels after the surgery was 1.6 gm% in group A and 0.4 gm% in group B and this difference was statistically significant. Also, the drop of serum protein after surgery was 8.6% gm/dl in group A and 1.0 gm/dl in group B and this difference was significant. Conclusion: There was no significant difference in the take of skin graft with or without removal of granulation tissue in our study. Background: Burn infections are the major causes of morbidity and mortality in burn patients. The infectious agent could be both Gram positive and Gram negative bacteria. The aim of this research was to study the bacteriological profile of burn patients and anti-microbial susceptibility pattern of their wound isolates. Methods: This cross-sectional study was conducted in Kirtipur Hospital, Nepal from January 1st to March 31st 2019. A total of 109 burn patients were included and their wound swabs were collected at the time of admission. All isolates were identified by standardized microbiological procedures. Anti-microbial susceptibility tests were performed by Kirby Bauer disc diffusion method. Results: Of 109 burn wound samples, 56 (59%) yielded culture growth and the Gram negative isolates were more common than Gram positives (77% vs. 23%). The most common organism isolated was Klebsiella spp. (25%) followed by Acinetobacter spp. (21%) and Staphylococcus aureus (18%). Most burn wound isolates were of resistant strain. Particularly, highly resistant strain of Acinetobacter spp. were isolated, most of which were sensitive to Tigecycline, Polymyxin B and Colistin only. Conclusion : Gram negative isolates were common in burn wound isolates and the antibiotic susceptibility pattern was different for different organisms. The use of antimicrobials should be judicious to further not escalate the problem of antimicrobial resistance in the healthcare settings.
